Health Form

A health form is required for each child attending camp. This form does not need to be signed by a doctor, but does need to be filled out and signed by a parent or guardian.

Please note the following:

All medicines, prescription and over the counter, must be held by the health director, unless contra-indicated (in which case we must know about it). If you have any questions, please feel free to ask.

Please send any medicines in the original container. Prescription medicines must have the child's name and dose. We are unable to dispense any medicines differently then they are prescribed, or to anyone for whom the medicine is not prescribed.

You do not need to send Tylenol, bandaids, antibiotic ointment or other "common" medical items. We stock these in our infirmary.

Our policy is to notify you in any situation where your child must spend a night in the infirmary, or needs more advanced medical help then we can provide (they must go to the hospital or doctor). In these situations we will make every attempt to notify you as soon as possible (in non-emergency cases before we leave camp). We will also contact you if we have any questions in regard to your child's health care.

We want your child to have a safe and fun experience at camp. Please feel free to share anything with us which you think will make their experience better.
